The federal government has proposed a system to allow consumers to report medical mistakes and unsafe practices by health care workers.

Patients have useful information that could explain medical mistakes, officials say, and hospitals and some doctors are on board with the idea, according to and article in The New York Times. But there are concerns about malpractice liability and financial penalties, and one physician noted the importance of matching those patient reports with the medical records.
